The Central Utah Clinic works with most insurance carriers and plans. Please contact your insurance company before your first visit to make sure your office visit and treatment are covered, and please remember to bring your insurance card and co-pay with you to your appointment.
If your surgery is being covered by workers’ compensation, you will need to provide the name of your workers’ compensation company and your claim number. You will also be asked to provide your employer’s name, phone number, and the name of your company’s injury coordinator.
